With Worksite Wellness, you get all the resources of a brick and mortar medical clinic brought directly to your worksite. We’ll tailor a comprehensive medical surveillance program to your specific needs. Our services include those related to NFPA 1582 mentioned above, silica exams, physical testing, heavy metal labs, PPE fit testing, x-rays, audiometric exams, and much more. We safely maintain all of your team’s medical records, and provide you with quick access.

Please complete all medical questionnaires requested and upload to your Patient Portal as soon as possible.
Please bring a photo ID and any pertinent medical records such as: immunization records/CPAP results/specialists consult reports.
If you are having bloodwork completed as part of your surveillance program, we do request you fast for 12 hours for accurate blood glucose levels. If you forgot to fast or can’t fast, we will still draw labs and document as “NON-FASTING”. This will allow the medical provider to determine if any further testing is needed if any abnormalities arise on results.
Yes. We provide every employee with a link to our patient portal that will allow access to your full medical records with Worksite Wellness.
Your employer will inform you of any drug screening requirements as part of their company policy for pre-employment examinations. Annual exams do not allow for drug screening for most industries. If you are unsure about your company’s policy, please reach out to your company resource.
Yes. During a comprehensive physical examination, you will be asked to remove some clothing and change into a gown if needed. The gown will open in the front to allow us to perform an EKG. We will also need you in a gown for the chest x-ray. The physical examination does include hernia checks and there are age requirements for prostate exams and rectal exams as well. Please note that our staff and medical provider will ensure you are comfortable at all times and ensure privacy. Please let us know if you have specific questions or requests.
No, but please keep in mind the exam may require you to have labwork, chest x-ray, cardiac stress testing, etc, so please wear comfortable clothing. Please note that we do have paper and fabric patient gowns available if needed.
No. The employer’s (DER) Designated Employer Representative will only receive a clearance form alerting them of the employees examination findings (Pass/Fail/etc).
No. The employer is not allowed to discuss an employee’s medical history or medical record with the medical provider or clinical staff. Our staff will only provide the employer with a medical clearance for the employee based on the job description. All medical records/results will be kept in Worksite Wellness’s possession in our HIPAA compliant EMR and HIPAA complaint patient portal.
Please bring your glasses or contacts with you to your appointment. We conduct a comprehensive vision exam that requires near vision/distance vision/color/depth perception, so please bring bifocals if needed.
Please wear/bring hearing aides with you to your appointment. We will also request that you bring any recent audiogram records from your audiologist as well.
Please feel free to reach out to our clinical staff with any specific questions, but all immunization records/screening records are accepted. Bring any copies of records to your appointment and our clinical staff will review records and conduct screenings only if needed.
Please bring any records from specialists that you are seeing on a regular basis that you find pertinent to your medical examination. (Examples: Cardiologist/recent Cardiac Stress Testing/Neurology Consults/MRIs/etc).
Please reach out to our clinical team and we will assist you in rescheduling your appointment at one of our local clinics in Yorkville and Naperville.
If you are currently on light duty, we are not able to conduct your annual surveillance physical examination. The annual examination requires you to be at full capacity at time of exam. We will make arrangements with your employer to reschedule you for annual compliance once you are released from your treating provider. If you have any questions, please reach out to our team.
If you are sick, please reach out to our clinical team and we will assist you in rescheduling your appointment or to discuss symptoms with clinical staff.
